   Click on this link to retrieve the assessment document.  Then download it to your computer to complete the assessment.  When you are completed with the assessment, please e-mail it to mmas@er9.org. Assuming you will be discussing this at your November/December curriculum meetings, the completed form is due one week after your curriculum meeting.

5.  Request for Curriculum Work,
   Click on this link to retrieve the request document.  Download it to your computer and e-mail the request to mmas@er9.org.  I must have your proposal by Wednesday, November 26.  Please fill in the worksheets for each of the three districts and the Tri-district.  Do all your text entry in the Easton worksheet first. Type in the name of the committee in the cell called Committee.  The text will automatically fill in Redding, Region 9, and Tri-State.  When you state your objectives (tasks) and product(s) in the Easton worksheet, the text will automatically fill in for Redding, Region 9 and the Tri-district.
   
   For each of the three districts fill in the number of teachers and the total number of days for each district.  The Tri-district worksheet will automatically add the number of teachers and total number of days for each of the three districts.  I will fill in the curriculum rate columns for the three districts..

Curriculum General Forms Forms:

Click below to view the Curriculum (1116-116) Payroll Form.   This form has been redesigned to be used for summer work and other approved curriculum work.  For your convenience this new design is a .pdf format and allows you to type all your information you directly into the .pdf file.  After typing in your information you simply it out and forward the document to Marie Mas.  Please call Valerie at the central office if you have any questions or problems with this form.

• Committee Structure List -  - Use this form to compile your full curriculum committee lists as needed.

• Summer Work Name Collection Form.xls - p - See notes below:

  • After you click on the form above, you will need to do a file save as to save it to your computer.

  • This form is for the Chairs & Liaison of each committee.

  • Region 9 teachers contracts have changed and you no longer need to separate IA's and Chairs from teachers.

  • Use the above form to list your teachers whom will be performing summer work this year.

  • Unless provisions have been made to be paid from this years budget, no summer work can be performed prior to July 1, 2006.

  • Only one completed form should be emailed to Marie Mas with a cc to Valerie Calvanezi, per committee.
