1. Physical Form

9th, 11th and 12th graders -
Submit yellow form
(yellow form is for 9th, 11th and 12th graders ONLY)

10th graders -
Submit BLUE state mandated "Health Assessment Record"
(required for all 10th graders by law.)

NOTE: the State requires the 10th grade form be on BLUE paper.
If you use this downloaded version, you MUST print it on BLUE paper
for it to be accepted.

These forms are available from the Nurse's Office or the Athletic Office.

2. Emergency Medical Authorization & Student Data

This form is available from DataProcessing.

3. Athletic Code of Conduct and Permission to Participate are also required for your student to be eligible for participation in athletics for the school year.

If you submit these three forms, your student will be eligible to participate in athletics at any time throughout the academic year providing your student remains academically eligible, does not owe a fine, has signed the state mandated "Due Process" form, and has not been suspended or expelled. "Due Process" forms are distributed and signed during homeroom the first week of school.
