Field Trip - Form 1

The top section of Field Trip - Form 1 is used to grant permission to go on a field trip.
It is mandatory for attendance on a field trip.

The bottom section of Field Trip - Form 1 is required
ONLY IF a private vehicle is involved in the transportation.

Field Trip - Form 2

Field Trip - Form 2 is ALSO required IF a private vehicle is involved.
It concerns information about the driver and the vehicle's insurance.
